I'm sure a JAGMAN Investigation is already underway to determine the specific details of this tragic incident and there are MANY questions left unanswered!!
First... Why was the ship traveling so close to the tanker in the first place and what prevented ALL HANDS (all personnel on board) from reporting to their General Quarters station?!
Second.... Why was the berthing area closed off before ALL personnel was properly accounted for?!
Third... Did the Officer of the Deck (OOD) and Look Out Watches follow proper protocol, relating to the "Rules of the Road" of oncoming/approaching Vessels? Obviously NOT!!!
Forth... Who "signed off" on the OOD's final qualification? He/She was obviously NOT Qualified!!
There are dozens of more questions and I'm sure the lawyers for the family members of the deceased will hold the Pentagon accountable!!
Lastly, when I served with Surface Warfare Officers, I personally witnessed how the Surface Warfare Officer community literally "spoon feed" certain junior officers, mainly white, the answers, during their final oral qualification board. When they were "put to the test" on the bridge, they proved to be "incompetent" at best.
I was a ship-driver as a Deck Seaman and this incident could have been avoidable!!!
